To schedule a routine dental appointment please call our office at (606) 679-1402 during regular office hours or leave your name and phone number on our answering machine and we will contact you. It is very helpful if you have your dental insurance card handy when you call. Also, inform us of any pertinent health problems such as heart defects, diabetes, bleeding problems, etc.

For your convenience, we can mail or fax the patient information and health questionare to you prior to your appointment or you may download the form below. This will reduce "paper work" time, and allow you to be seen by the dentist more quickly.

Our office will call to remind you of your appointment 24 hours in advance.

Our office policy regarding dental emergencies is as follows: Please call as early as possible so that we can schedule you that day. If you call very late in the day we may be forced to appoint you as early as possible the following day. Our goal is to never refuse an appointment to someone in pain.
